Dear Clinic Director,
I'm applying for the Staff Physical Therapist position at Athletico Glenview. I'm a licensed PT in Illinois (#PT12345, active since 2020) with my DPT from University of Illinois Chicago and my OCS since 2023. For the last three years I've been a staff PT in a four-therapist outpatient orthopedic clinic, carrying 14-16 patients per day on a 60-minute initial eval / 45-minute follow-up cadence in WebPT.
My work in outpatient ortho has focused on post-surgical knee and shoulder cases — including building a return-to-sport protocol for adolescent ACL reconstruction that our clinic adopted as standard of care after a 28-patient case series showed 94% return-to-pre-injury sport at 9 months. I've also precepted three DPT students across the last two spring rotations (Clinical Instructor certified), and all three passed the NPTE on first attempt. I'm comfortable billing CPT 97161-97164, 97110, and 97140 accurately and handling the documentation load that comes with a 14-patient day.
Athletico's post-surgical specialization and structured mentorship for OCS-track PTs are specifically what's drawing me in — I'd like to deepen sports specialty work toward the SCS, and a clinic with a sports-medicine referral base is the right next step. The Glenview location also keeps me close to my current referral network, which I'd expect to transfer with me.
I'd welcome a conversation about caseload structure and mentorship timeline. Thank you for considering me.
Sincerely, Jamie Lee, PT, DPT, OCS Illinois PT License #PT12345
